We really need the ability to generate a new consultation form, when one is needed for a new treatment. At present we have the frequency set for clients to complete this annually, rather than for every booking, however there are some instances whereby the client may be booked in for a new/different type of treatment/service that requires us to send them a new consultation form. At present the current process means that the exisiting form shows as completed & sent if within the year, so you are then unable to send another or to ideally generate a new one Also it would be better if the consultation form could be made more editable, as currently most of the wording/questions can not be editied at all on the consultation form. For example 'Please check any health conditions which you have ever had previously or are now experiencing:, so for one of the options on the tick boxes is Pregnant so a client may tick this in the instance that they have been previously but are not currently. It causes confusion & means we have to clarify if they are currently, when they arrive. Also the Blood Pressure should be seperated in to either low or high, as there is only one box for both so it's not clear. We should be able to edit the form to better reflect whether the medical conditions are current or previous, rather than being lumped together.
