I need this as well. We have a "Policy Acknowledgment form required for all new guests to make sure they cant say they "didnt know" about policies and sometimes also need a color/esti/or hair extension form as well.

We would love additional or the option of attaching more than one form to a selected service. We have a extensions business and have a lengthy process and like to break up the consult process in two parts, one being a question and combined with a policy form in one (pre appointment) and then at consult in salon agreements, after assessing the hair they then would comply to our agreement form. would be great on the app if we could store extra forms and gain easier access and more options for the consult process and be able to add on their file.

STRONGLY AGREE, we are combining more and more modalities into one treatment, so much time waster by us and clients filling in multiples of the same thing.

Absolutely agree, Phorest need to look at the bigger picture here for the client/patient journey.

It's not just about sending a consultation form, it's about obtaining client consent once the client is in the clinic which requires a separate form to also be allocated to that same service. Consents need to be obtained at each visit, however, consultations are for new clients. Also, Aftercare forms need to be administered via email after each treatment. Come on Phorsest, Go Completly Paperless.

All customers require a consultation form and they now have to do a covid pre-screening form before each appointment, but I don't want to make them re-do their consultation form more than once a year! I'm quite surprised that this isn't a thing yet as it's kind of essential.

All of my appointments require a Medical Information Sheet to be Filled out along with the Service Consent. If I need to add the Medical Information to the beginning of each Consultation Form designated to each service and if they have two or three treatments (Filler, Botox, and Chemical Peel in one session), then they are filling the Medical Information out three times.

It would be helpful to send a short covid form to every client in advance of every visit and also be able to send them an online form specific to the treatment they are having on the day.
